Documents Required:
To secure your place on the waitlist and confirm eligibility, you will need to upload digital copies of the following:
Proof of Identity (one of the following):
- Valid passport
- Driver’s licence
- Birth certificate
Proof of Eligibility:
- Relevant visa or citizenship documentation
What Happens Next?
Once we receive your enrolment form:
- You will be added to our waitlist (subject to eligibility)
- When you reach the top of the waitlist and we accept your enrolment we will contact you and send you a health questionnaire, and book a 30-minute new patient appointment.
