Kia ora, my name is Nik, I am your Health Improvement Practitioner (HIP) at Connolly Street Medical.

 What is a Health Improvement Practitioner (HIP)?

  • A HIP works alongside your GP and practice staff to help with all health problems and stress.
  • Appointments are free for all ages, 20-30 min long, and you can book directly with the front desk team.
  • Nik works at the practice on Wednesday and Friday

Kia ora, my name is Tom, I am your Health Coach here at Connolly Street Medical.

What is a Health Coach?

I support people to make lifestyle changes that:

  • Support healthy weight management
  • Lower cholesterol blood sugars and high blood pressure
  • Support people to exercise
  • Support people to quit smoking
  • Improve mental health and stress

Tom works at the practice on Thursdays.

Consultations

With a Doctor

A usual consultation time is 15 minutes (standard fees apply). If you require more than that, please book a longer time. We charge an additional fee for longer consultations.

Our prices are the same for face to face and phone consultations.

Specialised services such as minor surgery, medicals, infusions, drivers licence have different prices, and are often dependant on time taken and consumables used. Please ask the Receptionist for an approximate cost if you are booking for a non-routine consultation.

With a Nurse

We have 3 nurses on most days

Cancellations or Missed Appointments

Cancellations

We understand that sometimes things may happen which mean that a patient cannot make their scheduled appointment.  If this happens we ask that patients notify Reception on 04 569 5601 as soon as possible, so we can make the appointment available for other patients.

Appointments cancelled less than 2 hours prior to the appointment will be charged the full appointment fee.

Did Not Attend

A text is sent as a reminder for a patient’s appointment, therefore if a patient does not attend their appointment, they will be charged the full consultation fee.

Late Arrivals

We ask that the patients let us know if they are running late for an appointment and it is the decision of the doctor or nurse if they are still able to see the patient.  It may mean that the patient will have to re-schedule their appointment.

Prescriptions

There are 2 easy ways you can order a prescription:

  • Phone our script line on 04 569 5601
  • Order through Manage My Health Patient Portal

Scripts will be ready for collection 2 working days after request

We send all prescriptions directly to your nominated pharmacy

Accounts

We ask for payment at the time of your appointment. We acknowledge this is not always possible so we encourage you to set up a regular automatic payment. We do not allow an account to build up without this in place. We add a $10 Administration Fee to all accounts not cleared by the end of the month (and where there is no regular automatic payment set up)

Debt Collection Policy

Results

For all laboratory and radiology test results, you will be contacted by one of our team if your results require further treatment to follow up. We encourage all patients to phone and talk to one of our Nurses about any test results.

By signing up to Manage My Health Patient Portal you can see your results online once the GP has signed them off.

Cell Phones and Audiovisual Recording of Consultations

We appreciate that you may need to be in contact with others while at the practice but we ask that your cell phones or other mobile device is put on silent or turned off during your consultation. If you wish to record your consultation this can only be done after discussion and with the consent of the doctor or nurse.

Audio Visual Recording of Consultations

The recording of medical consultations is becoming a more frequent occurrence.

If you wish to record your consultation we ask that you discuss this with your Doctor or Nurse and seek their consent.

Connolly Street Medical is happy to assist patients and their support persons to better understand their medical conditions and recognize that recordings of the medical consultation may help in treatment and management considerations being discussed.

The covert or secret recordings of a medical consultation would be seen as a breach of the trusting relationship that must exist between the patient and the Doctor or Nurse.

Any recordings whether of an audio or video nature, taken without the consent of the Doctor or Nurse would be regarded as a breach of that trust. Consequently, Connolly Street Medical would reserve the right to end the relationship between the parties. This means the patient’s enrolment with Connolly Street Medical would be ended and all further medical care would have to be sought from another Health Care Provider.

Occasionally our doctors may ask for your consent to record consultations for their ongoing professional development. There are strict rules that apply to this process and would be explained at the time consent was being asked of you.
